Add a Site to the Great Site List

This is a form used to define a new site, which will be added to the great site list.

Please answer the questions presented below. Pressing the Submit button will cause the contents of this form to be emailed to me. I will take this message and create a link in the Great Site List.

The form itself is designed to record information about one site per submission. If you would like to post many sites into the directory (or any other list we track), and can submit that information in the form of a database, send a message to Mark@TheCoffeePlace.com who will be glad to setup some sort of database import utility to simplify the process.

Please be as complete as possible. Any fields left blank will not be displayed on the web page.

Enter the Name of the Site
** If multiple locations are to be listed in this city, make sure the names are distinct. (ex Zoo, Public Zoo, etc)
Enter the City where the Site is Located.
** This is used as a grouping mechanism within the Country-State page
** If you enter a city that is not listed, the list will be updated to accomodate your entry
Enter the Country and State where the Great Site is located. Use a format like USA - CALIFORNIA
** This is used as the primary sort - A page will be created for each different Country/State code
Enter the Address where the main entrance to the site is located (If possible)
** Supply the address and city state zip information
Enter Web address where a websurfer can surf for additional information.
** Use a complete web address like http://www.thecoffeeplace.com
Enter Email Address, a websurfer can use to Send Email
** For example mark@thecoffeeplace.com
Enter Site or Attraction type. (For example Zoo, Park, Fair, Tourist Site, Tourist Shop, Lake, Historic). Eventually this will be converted into a combo box.
Describe the environment. (For example Outdoor, Mall with 300 stores, Natural Zoo.)
Admission or expected price to spend (US or Equivalent).
Enter any additional information that should be posted on this page.
** As this questionaire expands, information in this field will be moved to any applicable new fields.
